A series of studies led by cardiologists at the Universitair Medisch Centrum Groningen (UMCG) indicates that low-dose digoxin may play a vital role in managing heart failure. The findings, which were presented at the ESC Heart Failure Congress in Barcelona and published in Nature Medicine and the Journal of the American Medical Association (JAMA), suggest that the inexpensive drug can reduce hospital admissions for heart failure by approximately 25%.
Heart failure, a condition affecting over 500,000 people in the Netherlands, occurs when the heart struggles to pump blood effectively, leading to fatigue and breathing difficulties. Current clinical standards rely on a combination of four medications, often referred to as the "Fantastic Four." Researchers investigated whether adding digoxin—a drug costing less than ten cents per day—could provide additional therapeutic benefits.
In a randomized trial involving 1,000 patients across 43 Dutch medical centers, participants received either a low dose of digoxin or a placebo alongside their standard treatment over three years. While initial data on cardiovascular death reduction did not reach statistical significance, a meta-analysis combining these results with earlier studies confirmed a meaningful benefit, particularly regarding the frequency of hospitalizations.
The research also highlighted the potential risks of discontinuing the medication. A follow-up study of 600 participants revealed that those who stopped taking digoxin experienced a higher rate of health complications within six weeks compared to those who had never been prescribed the drug. Experts noted that low-dose digoxin functions by suppressing stress hormones like adrenaline, which helps reduce strain on the heart muscle, rather than forcing it to work harder as higher doses once did.
Given its affordability compared to newer, more expensive heart failure treatments, researchers believe these findings could influence future clinical guidelines. The research was supported by a 3 million euro grant from the Hartstichting and ZonMw.
